Your First Visit
We want your first visit to be welcoming, comfortable and fun. We ask our new patients to fill out a registration sheet, HIPPA form and a medical history form. These forms help us avoid possible complications and interactions with medical conditions and medications. For your convenience, please download the forms on the right side of the screen. We are excited to be your Kilmarnock dentists!
X Ray and Record Release
If you are a new patient and have had a full set of x-rays taken within the past year or a panoramic radiograph (a large film where the machine moves around your head while you stand or sit), we would like to have and review these x rays prior to your first visit.
If you have not had x-rays taken within a year, a full set of x-rays may be taken on your first visit. In order to provide adequate dental treatment, we will be unable to do an exam and cleaning without current radiographs.

About our Doctors
Dr. Daphne Papaefthimiou is an alumnus of the University of North Carolina where she earned a BS in Biology and attended the School of Dentistry at Creighton University in Omaha Nebraska. Dr. Papaefthimiou maintained a faculty position in the department of Prosthodontics at Creighton University for two years as well as private practice. In her spare time (which is limited with two children and a busy practice!), Dr. Papaefthimiou enjoys watercolor painting, gardening, running, and exploring the many historical sites in Virginia.
Dr. Steven Short earned a BA in Mathematics at the College of the Holy Cross in Worcester Massachusetts and a MS in Biomedical Engineering at the University of Connecticut before joining Dr. Papaefthimiou in Omaha where he completed his dental training at Creighton University as well. When he has free time, Dr. Short enjoys tinkering on cars and running. Drs. Papaefthimiou and Short have two children who keep them busy with sports and activities.
